HIPAA Disclosure

To the extent you submit protected health information to an audiologist or other hearing services provider as a healthcare provider,  the hearing services provider is likely to be considered a Covered Entity under HIPAA.  You acknowledge that My Hearing Helper LLC is serving a Business Associate to the Covered Entity pursuant to a Business Associate Agreement.  My Hearing Helper LLC receives monetary compensation from the Covered Entity for generating sales.  Such compensation may be derived in whole or in part from information you submit, including protected health information.  By submitting this information, you hereby consent to this use.  You also consent to My Hearing Helper LLC contacting you about additional products, services and treatment options.  This consent can be revoked per the terms of My Hearing Helper LLC’s Privacy Policy.
